In addition to helping ease pain and other symptoms, your professional hospice team:
- Works with you, your family and your physician to provide the medical, psychosocial, emotional and spiritual care you need, when and where you need it. Our nurses, pastoral (spiritual) counselors, social workers, hospice aides, trained volunteers, bereavement experts and others stand ready to help 24/7.
- Allows you and your family to make your own decisions, respecting your beliefs and goals.
- Helps you and your loved ones through your journey.

Important Information & Referrals
A member of Continuum Care Hospice is available to speak or meet with the patient and family members/caregivers to discuss Hospice services and provide Hospice resources if needed.
